Attention Parents of Blue Fish Pediatrics:
We are pleased to announce that we will be having an H1N1 flu vaccine clinic this Saturday, December 12th, 2009. If you have not yet had a chance to receive the vaccine, this is a great opportunity to get it. Vaccinations will begin at 7:00 AM. As of now, this flu clinic is specifically for FIRST-TIME VACCINES ONLY; if you have already received the first dose and require a booster vaccine, please contact our office and stay updated on booster vaccine availability.
We are making the H1N1 information packet available in advance, along with all the necessary paperwork, so that you may review them before you arrive. Please download and complete all necessary paperwork in order to ensure that your registration and check in process goes smoothly.
